Purpose of Position

The Human Trafficking Outreach Worker provides intensive case management and referral services to First Nation, Inuit and Métis individuals who are involved in, or transitioning out of human trafficking.

 

Some of the responsibilities of this position are:

  • Meet clients in hospital, treatment, detention or other institution in/out setting;
  • Complete thorough intake and assessment of client needs;
  • Develop goal focused transition plans with client;
  • Coordinate involvement of internal and external resources to be mobilized to execute effective support;
  • Coordinate supports for complex issues in order to ensure safety;
  • Provide outreach case management services;
  • Provide education and increase awareness of mental health and addiction resources available for Indigenous individuals;
  • Promote Indigenous beliefs, values and traditions that enhance health;
  • Evaluate program process;
  • Develop supportive relations with clients, family members, institution staff;
  • Advocate for clients to ensure quality and timeliness of service and supports;
  • Create and maintain appropriate confidential records and client files, complete reporting requirements.

 

Qualifications:

  • Post secondary completion in Social Work, Social Services or other applicable program of study;
  • Minimum of 3 years of case management experience, crisis intervention and outreach;
  • Experience in case management, database/records management, assessment and interview of clients;
  • Knowledge of social, economic and health issues relevant to the urban Indigenous community;
  • Demonstrated ability to communicate effectively with Indigenous clients, including a sound knowledge and understanding of the issues that impact on culture in contemporary society;
  • Evidence of strong organizational and negotiating skills and the ability to handle sensitive issues with tact and diplomacy;
  • Effective interpersonal and conflict resolution skills, positive and flexible member of a team who thrives under pressure and change;
  • Current CPR-First AID level C, ASIST, Non-Violent Crisis Intervention or willingness to obtain;
  • Proficiency with Microsoft Office tools, particularly Excel, Word and PowerPoint;
  • Ability to work flexible hours including evenings and weekends;
  • Clear criminal reference check to work with vulnerable population is required.
