Position Overview

The Community Health Worker (CHW) is a key member of the Primary Care Health Outreach team, with a primary focus on health promotion and chronic disease education. This role supports the Wabano Mobile Primary Care Outreach Van, assists with clinic intake, and provides community-based education and navigation related to chronic disease prevention and management (e.g., diabetes, cardiovascular disease, respiratory conditions).

This is a front-line, community-focused role, ideal for someone who enjoys engaging people where they are, building trust, and supporting long-term health and wellness through education, advocacy, and connection to care.

Key Responsibilities

Reporting to the Team Lead, Primary Care Outreach, the CHW will:

Outreach & Mobile Services

  • Drive and coordinate the Wabano Mobile Primary Care Outreach Van, ensuring it is stocked, safe, and prepared for community outreach.
  • Provide on-site outreach support including client registration, coordination with clinical staff, and creating a welcoming, culturally safe environment.

Chronic Disease Health Education & Promotion

  • Deliver health education and wellness promotion activities focused on chronic disease prevention and self-management, including topics such as:
    • Nutrition and food security
    • Physical activity and mobility
    • Medication understanding and adherence
    • Stress management and lifestyle factors
  • Support clients in understanding their health conditions, screening results, and care plans in a clear, respectful, and culturally relevant way.
  • Participate in and assist with community health events, workshops, and outreach initiatives focused on prevention and wellness.

Navigation & Advocacy

  • Provide system navigation and advocacy, helping clients connect to primary care, chronic disease programs, mental health supports, housing, and social services.
  • Assist clients with clinic intake, forms, referrals, and follow-up appointments.
  • Act as a trusted liaison between clients and care providers to reduce barriers to accessing services.

Documentation & Collaboration

  • Maintain accurate and confidential documentation in the Electronic Medical Record (EMR).
  • Contribute to program tracking, evaluation, and reporting related to outreach and health promotion activities.
  • Work collaboratively within a multidisciplinary team, including clinicians, outreach staff, and cultural support workers.

Required Qualifications

  • Indigenous candidates who meet the qualifications will be given priority.
  • Postsecondary education in Community Health, Health Promotion, Social Services, Indigenous Studies, or a related field, or an equivalent combination of education and experience.
  • Minimum 2 years’ experience in community health, outreach, health education, or social services.
  • Experience supporting individuals with chronic health conditions and/or delivering health education is a strong asset.
  • Valid G Class Driver’s License, clean driving record, and over 25 years of age (required for company insurance).
  • Knowledge of Indigenous cultures, traditions, and social determinants of health.
  • Experience working with Indigenous communities and/or other marginalized populations.
  • Strong interpersonal and communication skills, with the ability to explain health information clearly and compassionately.
  • Comfortable with basic computer use and data entry (EMR experience an asset).
  • Ability to work both independently and collaboratively in a fast-paced, community-based setting.
  • Physically able to lift, organize, and transport outreach supplies.
  • Willingness to be vaccinated and comply with infection prevention and control protocols.
