There are over 40,000 Indigenous people in our Nation’s Capital, and Wabano serves more than 15,000 of them each year. Wabano is a cultural hub for Indigenous people in the National Capital Region—bringing together traditional knowledge, community wellness, and accessible health services –- but health care is our foundation.

Wabano’s medical team expanded this year and now includes: 3 Family Physicians, 3 Nurse Practitioners, 2 Registered Nurses, 2 Medical Receptionists, a Medical Administrator, a Social Worker, and 2 Community Outreach Workers. Our medical team works collaboratively with Wabano’s Health Promotion and Mental Health teams to provide quality culturally-based care.

As part of the expansion, Wabano will begin offering health outreach through a mobile van and community events and workshops.

Wabano is seeking an experienced and motivated healthcare leader to develop and oversee the new Health Outreach program. This role will assemble and supervise a team of healthcare professionals, create and implement an innovative outreach plan, and build strategic relationships to advocate for Indigenous health equity.

RESPONSIBILITIES

Reporting to the Client Engagement Director, the Health Outreach Team Lead will supervise a team of three: Wabano’s two Community Outreach Workers and Social Worker and collaborate with the Registered Nurses and Health Promoters.  Responsibilities include:

  • Lead Outreach Program
    • Create and implement a plan for the Medical Outreach Van that engages the community.
    • Ensure seamless, efficient, and culturally safe service delivery across mobile and community sites.
    • Review and communicate medical directives with the team to ensure understanding and compliance.
    • Maintain accurate data for submission to funders
    • Create a responsive outreach plan based on:
      • Identifying where Indigenous populations are most densely located
      • Where there are the largest service gaps,
      • Feedback from healthcare partners: Indigenous organizations, health agencies, community leaders (including Elders and traditional knowledge keepers).
  • Lead and Supervise Team
    • Supervise and support the Health Outreach team.
    • Foster a positive, collaborative team environment grounded in Indigenous values of respect, reciprocity, and community care.
    • Monitor staff performance, coaching and supporting to meet both funder targets and community needs.
    • Provide guidance to your team on culturally-grounded health promotion strategies, ensuring that your team utilizes Indigenous-produced health education and communication materials.
  • Partner and Represent
    • Represent Wabano at Ontario Health Team and community health planning meetings.
    • Strengthen relationships with hospitals, health networks, and partner organizations.
    • Provide updates, reports, and recommendations to senior leadership and the CEO.
    • Promote culturally safe practices and advocate for systemic improvements in Indigenous health.

REQUIRED QUALIFICATIONS

  • Degree or diploma in Health Sciences, Public Health, Indigenous Studies, Social Work, or a related field; or an equivalent combination of education and experience.
  • Minimum 2 years of experience in health program coordination, outreach, or management.
  • Proven experience working with Indigenous communities, organizations, and leadership.
  • Strong understanding of Indigenous health determinants, cultural safety, and traditional wellness approaches.
  • Excellent communication, relationship-building, and community engagement skills.
  • Experience supervising multidisciplinary teams.
  • Valid Driver’s License and ability to travel to outreach sites.

ASSETS

  • Indigenous candidates will be given first preference if they meet the minimum requirements.
  • Knowledge of Indigenous languages, teachings, and community protocols.
  • Experience working with Ontario Health Teams (OHTs).
  • Understanding of data-informed health planning and evaluation methods.

SALARY RANGE

$78,000 – $81,000 plus benefits
